Community Children's Staff Nurse

Job summary

This band 5 role is suitable for a newly qualified children's nurse or children's nurse ready to move from hospital to community nursing, looking to work within the community children's nursing services. This role provides an opportunity to consolidate learning and gain autonomous clinical skills and expertise across nursing pathways, working in community settings. This role also provides opportunities to rotate within the wider community children's nursing service as part of a band 5 rotation/development to develop clinical skills and expertise within specialist school nursing team.

Main duties of the job

  • Responsible for the delivery of nursing care for children and young people (CYP) in the London Borough of Newham. To play a key role within the Community Children's Nursing services (CCNS) in the ongoing assessment and evaluation of the programs and plans of care.
  • Work in partnership with other community based statutory, voluntary and third sector organisations to deliver seamless services for children, young people and their families.
  • This is a development post that would be suitable for a newly qualified Children's Nurse or children's nurse moving from hospital into the community setting.
  • The post holder will hold a shared caseload with oversight from a senior member of the nursing team, within the scope of competencies in the nursing workbook and competency handbook for community children's nursing service.
  • This post will include rotation opportunities within CCNS - including specialist school nursing and our nursing pathways.

  • To work as part of the Community Childrens Nursing Service to ensure the ongoing assessment, planning, implementation and evaluation of individualised evidence based care.
  • Develop with other team members effective liaison with local statutory, voluntary and third sector organisations. Providing contribution to multidisciplinary assessments for children and young people. Attendance and contribution to multi-disciplinary, multi-agency meetings and discharge planning meetings.
  • Participate in the education, training and monitoring required to meet care needs of children and young people in their home, and community settings including schools, nurseries.
