Job summary
The purpose of this role is to be an active member of the multidisciplinary team in Bow and Poplar Neighbourhood Mental Health Team
The post holder will provide occupational therapy assessment and intervention to people in the Bow and Poplar neighbourhood referred to mental health community services.
To provide sessional brief input to community mental health service users of the Duty System.The role has particular focus on service users' recovery and social inclusion goals, via both individual and group work. There is care-co-ordination responsibilities. The success of the post-holder will be measured by service user outcomes and satisfaction as well as the degree to which OT input facilitates the wider service in supporting service users to step down from requiring secondary mental health care support.
Main duties of the job
To support service users' recovery by optimising independence, improving occupational performance and increasing social inclusion. Recovery intervention plans will support step down from secondary mental health services to community based universal services. Plans will be mindful of reducing costs to the service and partners and actively supporting move on plans through from supported housing and facilitating timely discharge from an inpatient settings where possible. Therefore complex decisions around prioritisation may need to be made and reviewed from time to time. It is essential that the OT resource is directed to where it is needs most and it makes the biggest difference.
