Job summary
This post holder will work with the Clinical Lead Pharmacist (Community Health Services Newham), to provide an appropriate level of pharmaceutical support to the services based at East Ham Care Centre and within Community Health Services Newham. The main focus of this role is providing a pharmacy service to 3 inpatient wards, including one providing intermediate step-down care and two providing care to elderly patients with complex mental health problems associated with degenerative brain disorders such as dementia. There will also be opportunities to extend the role into working with other Community Health Services such as District Nursing teams.
Main duties of the job
- To deliver a comprehensive medicines optimisation service to inpatient wards and community health service teams, and when necessary to provide this service to other wards across ELFT. This is to be in line with the ELFT clinical pharmacy standards.
- To provide and develop pharmacy services within services based at East Ham Care Centre, in line with local and national objectives.
- To provide input into policies and procedures affecting the use of pharmaceuticals at both local and trust wide levels under the direction of the Clinical Lead Pharmacist CHS Newham and the Chief Pharmacist.
- To work towards providing pharmaceutical support to the Extended Primary Care Teams (EPCTs) and other services within CHS Newham, liaising with the multidisciplinary team (MDT) as necessary.
- To work with the Clinical Lead Pharmacist to undertake audits to evaluate and improve services alongside Trust and local Newham Directorate priorities.
- To work towards quality improvement initiatives in line with the trusts QI vision and with direction from the Clinical Lead Pharmacist/Chief Pharmacist.
- To deputise for the Clinical Lead Pharmacist, CHS Newham when absent.
- To take part in the bank holiday and Saturday service rotas in the Trust's main dispensary at Mile End Hospital as per current departmental practice. Optionally, may also take part in pharmacy on-call service (Emergency Duty Cover).

They will support with quality improvement projects, provide professional expertise, advice, support and training on policies and practice to healthcare professionals in the directorate. The postholder should be an enthuiastic and committed pharmacist, must be able to show compassion and respect for people with mental health conditions and be driven to improve their care through medicines optimisation.
