Community Mental Health Nurse

This is a fixed-term (12 months) community nursing and care coordinator role within the diverse Bethnal Green and Globe Town Neighbourhood Mental Health Team.

To make assessments within Health (SAP/CPA) and Social Care (Care Management) frameworks.

To set up, implement and review appropriate care plans and to support and advise relatives and care givers.

Undertake comprehensive assessments, including risk assessment, effectively and efficiently.

Formulate and agree crisis safety care plans and to support and advise relatives and care givers.

Be rotational and flexible, working according to the needs of the CMHT service and Neighbourhood Team.

Main duties of the job

Develop and maintain a high standard of practice based on assessment through the use of pscyho education, the Recovery Model and supportive clinical management.

Liaise and establish good working relationships with referring agents, colleagues in Primary Care services, Social Services, Community Mental Health Teams, Assertive Outreach, In-Patient services, Criminal Justice, Voluntary Sector and other agencies as appropriate.

To work with the multi-disciplinary team in developing plans that offer supportive crisis care in the least restrictive way.

Be responsible for the maintenance and accurate recording of patient information in the documentation systems and to ensure the effective dissemination of service user information throughout the multidisciplinary team.Ensure compliance with the requirements Safeguarding Adults and Children's policies. To ensure that staff have the required knowledge and skills to work within these procedures to support the safety of vulnerable adults and children.

Have an understanding of the KPI's for the crisis service and ensure compliance with Trust CQUINs

Ensure own compliance with mandatory training is maintained.

Ensure full potential through supervision, appraisals and performance management.

Ensure that you are suitably skilled in addressing the physical health needs of service users.

  • RMN Educated to degree level
  • -Mentorship Training or equivalent
  • Recognised post graduate qualifications in care of the adult with mental health problems
  • oEvidence of regular updating and ongoing development of role and skills

Experience

  • Significant post-qualification experience, which must be within mental health services .
  • Experience of working within a multi-disciplinary environment/community experience.

  • oAbility to manage and co-ordinate professionals, teams and services and mobilise provision where appropriate. . Assessment skills and the ability to compile care plans in partnership with service users, carers and others
  • o Assertiveness and engagement with other professionals, able to make reasoned arguments and represent own professional views and perspectives
