Job summary

Community Mental Health Meaningful Intervention Practitioner

We are looking to recruit an innovative and independent practitioner who will be provided with an opportunity to work in a developing and nurturing team. You will work in multi-disciplinary recovery based mental health services and will be a core member of the multi-disciplinary team, participating in the provision of high quality mental health meaningful interventions. The role includes working closely with our community partners, including GP, staff and voluntary sector.

The team's vision is to move work from clinical settings into the community and focus on Newham's population health needs to dissolve boundaries between health and community.

Main duties of the job

To provide a high quality and effective mental health support and care to Newham residents. To respond to the needs of adults with mental health needs and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances. To explore need using a person centred approach and effectively assess and manage risk. To plan and deliver outcome meaningful interventions linked to promoting independence & social inclusion. To act as an allocated worker taking responsibility for the provision of community services to mental health services users and carers.

  • Professional registration: Mental Health Nursing, Social Work, Occupational Therapist or Arts Therapists qualification
  • Evidence of continual professional development
  • Post qualifying experience of delivering relapse prevention work and specific psycho-social interventions

Desirable

  • Post qualifying certificate/diploma/Msc in mental health.
  • Approved Mental Health Professional

Experience

Essential

  • Experience of working in a mental health community team as a qualified member of staff implementing the recovery model of care.
  • Experience of making full psycho-social assessments and writing reports.
  • Experience of working as part of a multi-professional team

Desirable

  • Experience of providing support and education to carers/families and supporters
  • Experience of working in a multi-racial inner city area
