They will also support other clinicians in signposting and making onward referrals to other services. Playing a key role in enhancing inter-agency relationship and interface, the post holder will be supported, where appropriate, to attend team meetings and deliver collaborative projects/ interventions in identified settings as guided by the service's needs.
About us
Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive - so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.
The service offers hybrid working and therefore, there will be flexibility to do some remote working. This is subject to change in line with service needs.
