As part of the new primary care directorate within ELFT our team will be involved in leading triple aim population health quality improvement work. Quality of care is our top priority. Main duties of the job
- Under the direction of the Practice Nurses and ACPs, undertake basic nursing and technical tasks to provide a comprehensive nursing service for Practice patients.
- Liaise with the Diabetic Team and be responsible for the administration of diabetic care plans.
- To provide a phlebotomy service for the practice population
- Undertake routine diagnostic nursing tasks such as, ECGs, blood pressure checks, ambulatory blood pressure monitoring, blood glucose monitoring, routine heights, weights and urinalysis
- Undertake spirometry (if trained to do so)
- Use patient contacts as opportunities to promote health education, including education on practice systems such as obtaining a blood result.
- Participate in health promotion projects such as No Smoking Day
- Take responsibility for the administration of diabetic care planning, coordinating patient results and clinical observations and ensuring the care plan is sent to the patient in time for their annual review
- To participate and administer the NHS Health Check Programme
- Provide vaccination to relevant cohorts (if trained to do so)
