Crisis Pathway Pharmacist

Job summary

Are you anenthusiastic, motivated and committed pharmacist? If so we would like you to join our mental health pharmacy team!

You will need to have the skills to be able to work within the wider multi-disciplinary team of nurses and doctors and other allied healthcare professionals and managers. You will want to develop and improve the reliability and quality of our clinical pharmacy service. You will need to have specific experience working with pharmacy teams and patients within the mental health setting. You will want to reach targets set by the Trust and by the pharmacy service. You must be self- motivated and able to motivate others.

Our dynamic and creative ELFT pharmacy team provides pharmacy services to a wide range of community and inpatient service users in Luton and Bedforshire, City and Hackney, Newham and Tower Hamlets. This includes children and young people to adults of working age, older adults, and forensic patients.

You will lead on the provision of pharmacy services in Hackney Crisis Team. You will also provide support to the medicines management technicians and pharmacists, provide operational support where required, provide a clinical pharmacy service to inpatient wards and work with the multidisciplinary team to ensure the delivery of a safe and effective service to mental health patients in our directorate.

The post is 37.5 hours worked over 5 days, Monday-Friday. There may be an opportunity to build hours on BANK outside the contracted hours.

Main duties of the job

Work towards health promotion and prevention and comprehensively assess and manage patients for risk factors and vulnerabilities.

Lead on Medicines Optimisation initiatives within the Crisis Pathway

Provide a direct link between ELFT and Homerton pharmacy teams.

Provide Education and Training to MDT and pharmacy team members

Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions, which will include psychological therapies and prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions

Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimize health outcomes and resource use, and provide direct support to staff, patients and clients

Work in partnership with patients and carers promoting a recovery focused model of care

Provide input into policy and procedures affecting the use of pharmaceuticals at both local and trust wide levels under the direction of the Lead/Advanced Pharmacist.

Ensure Parity of Esteem between physical and mental health and promote this agenda amongst the wider MDT

Promote the trust Medicines Safety Agenda in collaboration with the Lead Pharmacist and Medicines Safety Officer

Essential

  • Masters Degree in Pharmacy
  • Member of the General Pharmaceutical council and/or Royal Pharmaceutical society
  • Psychiatric qualification or significant experience in psychiatry

Desirable

  • Supplementary or Independent Prescriber (or willing to work towards this)

Knowledge

  • Up to date knowledge of current white papers/ national/ government reports affecting psychiatry
  • Drugs used in psychiatry.
  • Aware of effect of political changes on healthcare
  • Sound knowledge of drug management of Mental Health disorders including national guidance
  • Clinical and critical appraisal skills
