Job summary
East London Community Forensic Service (ELCFS) is an enhanced multidisciplinary service providing care, treatments and support to service users in the process of and following discharge from secure care services. ELCFS also provide a liaison and consultation service to secondary care mental health services. ELCFS serve/cover both ELFT(Newham, City and Hackney and Tower Hamlets) and NELFT(Barking & Dagenham, Havering, Redbridge and Waltham forest)localities.
An exciting opportunity has arisen for an experienced and motivated band 7 registered mental health nurse to join our expanding service.
This role will provide the successful candidate with the opportunity to work in and be part of the NHS England's innovative" specialist community forensic team model" that focuses on: 1) providing INREACH support to help service users move safely towards discharge, therefore reduce length of stay in secure care. 2) providing enhanced support in the community and so promote sustained recovery and quality of life in the community, reducing recalls to hospital.
The successful candidate will be responsible for managing a caseload of service users discharged from the forensic inpatient services into the community. You will act as a care coordinator for the allocated service users in designated locality and work collaboratively with relevant agencies, services, families and carers.
The postholder will be expected service to one or more of the localities covered by the service(e.g Newham)
Main duties of the job
Care Co-ordinate locality forensic patients that are discharged from forensic services into the community as agreed by the MDT sector team.
To undertake comprehensive specialist forensic nursing assessments of mentally disordered offenders in a variety of complex settings related to the forensic or community mental health teams to which they belong.
To plan, implement and evaluate care programmes that are developed which reduce the risk of violence and re-offending of the forensic patient in the community.
Engage in the quality improvement projects and use related approaches to think systemically about complex problems, develop potential change ideas and test these in practice using the Trust's QI framework
Be able to administer medication safely and in accordance with relevant policies and procedures.
To represent the forensic community mental health services at the Multi-Agency Public Protection Meetings and/or the local Mentally Disordered Offenders Group within your locality as required.
To provide specialist advice to the locality teams on how to effectively manage mentally disordered offenders in the community, provide support, and risk assessment/management with regard to their transfer and care within the community.
To provide packages of care and psycho-education for mentally disordered offenders and their families/carers that takes account of the complexities of family systems and their significant social networks.
