Job summary

We are looking to recruit a Dementia Nurse Specialist to join the three nurses already in post.

This is a unique opportunity to work as part of a dynamic team and pioneer the implementation of an innovative, responsive and future proofed model of care.

The City and Hackney Dementia Service will ensure everyone diagnosed with dementia is accounted for and they and their family /carers are supported at each stage of disease progression or as things changes.

It will provide a diagnostic service that registers, holds, tracks and cares for everyone diagnosed with dementia from diagnosis till end of life and who resides in City and Hackney or are placed by out of borough services by ensuring timely access to assessment, diagnosis, ongoing post diagnostic support and treatment, regular reviews, easy access to medical review and other services.

Main duties of the job

To be an active member of the multi-disciplinary team providing diagnostic and post diagnostic careand support to older people with mental health needs and individuals presenting with organiccognitive decline.To manage a caseload of service users and their carers / relatives supporting them through thediagnostic process, providing post diagnostic support and follow up.To actively monitor deterioration using a risk stratification tool - Activity of Daily Living, mood,cognition, general health and safeguarding)Actively engage people with Dementia and Cognitive impairment and their carers in all stages onintervention, ensuring that their views are acknowledged and taken into accountTo liaise and work closely with GPs, Specialist Palliative Care referrals, identifying when someoneneeds palliative approach, approaching end of life (EoL), the Integrated Independence Team (IIT)and Geriatrician Referrals)To liaise and work closely with linked Social Workers in Neighbourhoods to facilitate timelyprovision of social service input including formulation of joint crisis planTo promote the needs of service users with mental health needs and Dementia, providing trainingand psycho-education whenever possible.To conduct carers assessment and take action where required, including offering of support and referral toother services.

Experience

  • Previous specialist nursing experience at Band 5 and above.

  • Previous experience of independently conducting psychosocial assessment and planning care for service user with complex care needs
  • Previous experience of providing post diagnostic support for patients with dementia
