Job summary

East London Foundation Trust is looking for an enthusiastic and proactive individual to provide strong operational leadership and clinical experience to our Integrated Discharge Team. This is an exciting opportunity for the right candidate to take their clinical and management skills to the next level, leading this key team linking acute and community services.

The successful individual will be lead a ward based team consisting of nursing, therapy, social work and admin team members. You will be driving forward links with NUH wards and complex discharge team, as well as community services in ELFT. You will be working closely with health and social care partners to ensure safe and timely discharges and minimise barriers, improving patient experience across discharge pathways.

Main duties of the job

You will provide clinical and operational leadership to the ELFT Integrated discharge hub who undertake 'in reach' triage assessments for patients on the acute Wards at Newham University Hospital to ensure rapid discharge. Undertake generic IDH clinical assessment and coordination tasks and functions.

You will be responsible for developing and supporting nursing and therapy staff in the delivery of case management, planning and supporting discharge from acute into community services, ensuring all patients have a safe and timely discharge. You will work as part of an integrated team with NUH Complex Discharge Team, working closely with their Team Lead and NUH Associate Director Site Operations and Community Integration.
