Job summary

Band 6 Mental Health Practitioner Join our neighbourhood-based community mental health teams, where integrated health and social care come together to support recovery and empower service users. As a valued member of our multidisciplinary team, you'll play a crucial role in delivering recovery-focused care through needs assessments, practical support, and evidence-based interventions. As a Band 6 Mental Health Practitioner, you'll work as part of a dynamic multidisciplinary team, providing assessments, care coordination, and tailored interventions. Your role will focus on promoting sustainable, strengths-based recovery and reducing dependence on long-term services by enabling service users to take charge of their journey. Main duties of the job

Key Responsibilities:

  • Lead on managing a caseload of complex mental health service users as a care coordinator using Care Program Approach
  • Develop and implement person-centred care plans using evidence-based principles to meet individual needs.
  • Facilitate smooth transitions between services, ensuring timely and effective support.
  • Provide mentorship, supervision, and leadership to students.
  • Actively contribute to service improvement, taking ownership of defined projects and responsibilities.

Qualifications

Essential

  • Registered Mental Health Nurse
  • Evidence of continual professional Development
  • Trained Adult Safeguarding Enquiry officer or willing to undertake Enquiry Officer training.

Desirable

  • Mentorship Training or equivalent

Experience

Essential

  • Experience in a relevant field in Mental Health.
  • Experience of hospital or community caseload management.
  • Experience in working with a diverse community in a complex environment.
  • Experience of working as part of a multi-Professional or multi agency team
  • Experience of conducting holistic psycho-social assessments and providing comprehensive care and support plans based on individual need.

Desirable

  • Experience of working within an NHS/Local Authority environment and an understanding of the issues/ policies/ practice within these environments.

Knowledge

Essential

  • Understanding of the Mental Health Act 1983 and how the act is used in practice.
  • Understanding of the Mental Capacity Act (2005) and how the act is used to support service users and carers / relatives
  • Knowledge and understanding of the Care Act 2014 and how the is Act implemented within care provision.
  • Understanding of the principles of CPA and the Recovery Model.
  • Up to date knowledge of the current approaches to mental health nursing.
  • Up to date knowledge of the issues that impact individuals and carers affected by mental health and or cognitive impairment.
  • Knowledge and understanding of approaches to risk assessment and management.
