Job summary
- The post holder will be an experienced nurse or allied health professional, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.

- The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
- The post holder will build effective working relationships with other health care professionals.
- The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice
- Clinical Expertise: Demonstrate expert clinical knowledge and skills in the assessment, diagnosis, and management of patients. This includes conducting comprehensive patient assessments, formulating treatment plans, and prescribing medications as authorised.
- Collaborative Approach: Work collaboratively with the healthcare team to provide holistic care and ensure effective coordination of patient services. This involves regular communication, consultation, and collaboration with physicians, nurses, and other healthcare professionals
Main duties of the job
- Patient Education: Educate and empower patients and their families by providing clear and concise information about their condition, treatment options, and self-management strategies. Foster a patient-centred approach that promotes shared decision-making and patient satisfaction.
- Professional Collaboration: Engage in interprofessional collaboration and contribute to multidisciplinary team meetings, case conferences, and clinical audits. Act as a resource and liaison for other healthcare professionals to optimise patient care.
- Documentation and Record-keeping: Maintain accurate and up-to-date patient records, including electronic health records, progress notes, and treatment plans. Ensure compliance with legal and regulatory requirements regarding patient confidentiality and data protection.
