Job summary

We are recruiting to a new role as a Health coach to work across one of our Community Integrated Mental Health Services (CIMHS) and a Primary care network (PCN) in Newham. The purpose of this role is to support people to improve the way they manage their physical health when living with a mental health condition, based on their context and what is important to them. You will work with a small group of service users todevelop their knowledge, skills and confidence in managing their physical health and support them making changes in health-related behaviours.

You will work with people in the community, often at their homes and community venues. This is not a signposting role - you will need to be able to meet people where they are, physically and psychologically.

As this is a new role, you will be expected to share learning through case studies, data and themes emerging. As such, the successful candidate must be able to draw on qualitative and quantitative data.

The successful candidate will be kind, reflective and self-aware and will have a range of skills, knowledge and experience to draw on. Youmust hold a firm belief that people have untapped resources within them, that can be unleashed by providing high quality, non-judgmental support.

Main duties of the job

1. Provide one-to-one and group health coaching support for people with a serious mental illness (SMI), based on what is important to them, with the aim of:

a. improving people's knowledge, skills, confidence in managing their condition/s

b. Support people to feel empowered to manage and improve their health and health outcomes

c. support them in making changes in their health-related behaviour

d. Support them to understand the complex interplay between their mental and physical health

2. Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self- Management, modelling the coaching approach in their work.

3. Play a leading role in forging links and relationship with other services; including physical health services (such as diabetes care and other services provided by Community Health Newham), lifestyle services (stop smoking, weight management, exercise on referral, leisure centres etc.) and supporting access to these services for service users on their caseload and people open to ELFT mental health services in general.

4. Collect service user experience and impact of health coaching as part of the delivery of personalised care, using QI methodology where appropriate.

Education/ Qualification/ Training

Essential

  • Educated to degree level or equivalent combination of short courses and experience
  • Be willing to attend training with a nonclinical Supported self-management health coaching skills programme (minimum 4 days) by a Personalised Care Institute (PCI) accredited trainer or organisation prior to taking referrals.

Desirable

  • Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical Supported SelfManagement (SSM) Health Coaching through a PCI-accredited organization
  • Registered Professional in either Nursing (Mental Health), Social Work or Occupational Therapy or other relevant profession, with current registration with NMC, Social Work England or HCPC

Experience

Essential

  • Experience of working in health or social care care/community development setting or similar
  • An understanding of the biopsychosocial model of health and the social determinants of mental health.
  • Experience of communicating difficult, complex and sensitive information to a variety of people inc. service users and their carers

Desirable

  • Experience of using coaching approaches/frameworks and models or other helping strategies e.g. Motivational Interviewing
  • Experience of working in a multidisciplinary team
  • Experience of successful partnership working across statutory, voluntary and community sector

  • Interpersonal skills enabling therapeutic alliances to be developed and maintained with clients who have complex needs.
