The Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. Additionally, the service is responsible for the provision of a specialist day activity hub - CREATE.

An exciting vacancy opportunity has for a continuing care nurse post to join CLDS team.

Main duties of the job

This is the lead (sole) Continuing Care practitioner within the Tower hamlets Community learning disabilities Team for Adults with Learning Disabilities to undertake this specialist role.

To undertake NHS Continuing Healthcare Assessments for adults with learning disabilities on behalf of the North East London Integrated Care Board (ICB) in line with current National Framework and guidance.

To fully case manage a caseload of adults with learning disabilities and continuing healthcare needs including monitoring of placements, identification of new placements and services, development and review of care plans, preparation of additional funding requests and negotiation of care packages with families, organizations and the ICB.

Where an individual is found eligible for NHS Continuing Healthcare, to support families with the implementation of personalised care plans and management of personal health budget (PHB) negotiating funding arrangements regarding personalised care packages..

To find appropriate/urgent placements for individuals whose placements have broken down. To lead on Safeguarding investigations for Continuing Healthcare funded adults with learning disabilities.

Use clinical judgement and frequent reviews for individual where their placement needs have change, or the complexity of their health requires greater frequency of care.

Be able to manage challenge and conflict in regard to NHS Continuing Healthcare processes and recommendation/decision making.
