Job summary
City & Hackney Rehabilitation & Recovery Service provide care co-ordination for service users with severe and enduring mental health needs living in Hackney. In addition we provide regular reviews for all service users placed in funded accommodation, both within and outside of the borough and provide specialist case management for service users who are not care co-ordinated to facilitate moves back into borough. The teams focus is on supporting service users towards greater independence, building their confidence and ultimately enabling service users to step down to less supported or more appropriate accommodation. Team members work with providers ensuring that placements are delivering high quality care within cost efficient parameters.
We are looking for a motivated and flexible Community Psychiatric Nurse to join our well established specialist team. Experience and enthusiasm for working with individuals with severe and enduring mental health needs is essential.
The City & Hackney Rehabilitation & Recovery Service is a specialist community mental health team which offers care co-ordination for service users with severe and enduring mental health needs living in Hackney. In addition, we carry out regular placement reviews and offer case management for all service users placed in funded accommodation in order to facilitate moves.
Main duties of the job
To independently manage a caseload of allocated service users with complex mental health needs and act as a care co-ordinator and Case Manager to service users
To provide assessments of the service users placement, how their accommodation is meeting their needs and whether there are any unmet needs; presenting this to placement review meetings as required.
Where appropriate to support service users to move accommodations taking into account practical considerations and support needs through liaising with all relevant organisations and professionals i.e. accommodation providers, care co-ordinating team, GP etc.
Undertake the necessary training to provide cover in the clozapine clinic and to provide clinical cover to the clozapine clinic as and when necessary.
To undertake supervisory and appraisal responsibilities for allocated team members.
To act as fieldwork educator to students on practice placement.
To promote a service user focussed service in line with Recovery Ethos
