Job summary

We are excited to recruit to this newly created post for a Community Health Assistant Practitioner. You will be working with patients in the community supporting both the nursing and therapy teams, utilising a broad range of skills covering nursing and therapy competencies.

The successful postholder will be adept at working across teams and communicating with patients, their families, colleagues and other service providers. They will also be committed to providing high quality, patient-centred care.
