Job summary
We are looking for a psychologist to provide clinical leadership in the Rehabilitation provision in East London. This is a new leadership role across the three East London Boroughs combining psychology leadership in Newham and Tower Hamlets and City & Hackney with an additional clinical role in City & Hackney where the post holder will be based in the only Rehabilitation Team in the Trust. Applicants must be willing to travel between sites as this role covers the three boroughs of East London.
This post comprises of an existing clinical role in the City & Hackney Rehabilitation Team and a new role providing psychology leadership for the population requiring across our London Boroughs (City & Hackney, Tower Hamlets, Newham).The post holder will be responsible for the development and delivery of the psychological provision in the community mental health provision for people with rehabilitation needs (SMI/SMHP/ chronic +/- complex psychosis.) This is a part of a broader model of integrated community mental health care, centered around primary care networks as per the NHSE Community Mental Health Transformation.
You will be a key member of the MDT (nursing, medical, social work, occupational therapy, support workers) and will be expected to work closely with experts-by-experience, local communities, Community and Voluntary and Social Enterprises, to co-produce and co-develop services.
Main duties of the job
The clinical role involves providing psychology input for the well established MDT Community Rehab team working with people with SMI over long periods, supporting management of complex situations for the wider system and supporting the mental health supported accommodation pathway.
The team care coordinates 80 people (with prominent positive and negative symptoms, treatment resistance and poor activities of daily living function), carries our annual reviews for funded placements on behalf of the local authority and ICB, and provides liaison with many services especially inpatient liaison, facilitating timely disharges, panel consultations, support accommodation panels, forensic services and others.
The service carries out annual reviews of all funded placements (150 per year) on behalf of the local authority and CCG using a placement review tool, and also plans step-down. If required, the service also provides case management for transitions and moves, or when changes in needs are identified. The service also provides acute inpatient liaison and assessment to facilitate timely discharge by identifying placements and signposting housing panel processes swiftly. They have developed pre-panel consultation sessions for discussion about appropriate accommodation options for community teams (including forensic). They co-facilitate the Mental Health Supported Accommodation Panel, liaising with providers across the pathway and managing housing quota allocation.
