Job summary
The East London Community Forensic Service (ELCFS) is an enhanced multidisciplinary service providing care, treatment and support for service users in the process of and following discharge from the Forensic inpatient services in the boroughs of Hackney and Newham. This model of service is now being expanded to other ELFT(Tower Hamlets) and NELFT(Redbridge, Waltham forest, Havering and Barking & Dagenham) boroughs covered by ELCFS.
An exciting opportunity has arisen for an experienced and motivated Life Skills recovery worker (Band 4) to join our expanding service.
The successful candidate will play an active INREACH Role with the inpatient forensic services and intensively engage patients earmarked for discharge. You will focus on forming therapeutic, meaningful relationships with patients; you will use a range of interventions/activities to enhance their care and support with their transition back in the community. Main duties of the job
The post holder will focus on forming therapeutic, meaningful relationships with patients, they will use a range of interventions/activities to enhance their care and improve their inpatient experience. This will include:
-Work with patients on an individual and group basis through the recovery process.
-Assist in the delivery of therapeutic/ health promotion groups and activities for service users
-Assist in the delivery of health promotion activities for individual service users and groups of service users.
-Provide general support to all team members.
-Be prepared to participate in staff support groups/team away days
-Identifying and communicating to senior staff how health, safety and security can be improved within the team and throughout the service
The post holder will work with other members of the MDT to both organise and facilitate activities appropriate to the individual needs of the patient group looking into consideration capabilities, limitations and risk management.
