Job summary

An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse to join the Community Integrated Mental Health Services (CIMHS) South Team, central to the Newham Mental Health Transformation. The service aims to provide care to the service users of Newham, working in partnership with the Primary Care Network (PCN) structure in Newham and third sector provision. The service offer a flexible and accessible approach to working with service users to achieve their goals towards recovery.

We are looking for an enthusiastic and motivated Mental Health Nurse to join our dynamic and innovative team in CIMHS South. The role will involve working collaboratively with PCN colleagues, providing advice and support, in addition to carrying out assessments and interventions. In addition to a medical team, you will be working in partnership with OT's, Community Connectors, Peer Support Workers, Clinical Associate Psychologists and many more.

Main duties of the job

To link with a number of GP practices and with the community recovery team catchment area teams and Consultant Psychiatrists linked to the PCNs, providing Primary Care colleagues with advice and support on cases they are managing, including carrying out assessments at the GP's request.

To gather information from GP surgeries, CRT's and prepare in organising provision of assessment, treatment of service users with mental health needs in the primary care settings and to supervise primary care support workers including Peer Support Workers, Community Connectors etc. To be an active member of the multi-disciplinary team and be responsible for a caseload of people with mental health problems who are being managed by Primary Care, including the administration of Depot medication;

The MHN will promote effective psychiatric interventions and improved Care Pathways by supporting GP practices to manage Primary Care Mental Health needs. The MHN will support the CIMHS for Mental Health interventions (and Depot administration, where/if needed). The MHN will support robust and reliable referrals systems between the GP practices and secondary care as well as support the development of primary/ secondary care interface.
