Job summary

Cancer Pathway Coordinator - Band 4 - 16 hours

An opportunity has arisen for a Cancer Pathway Coordinator to join out team for a 6 month fixed term contract, covering a secondment.

This post will be integral to the tracking and expediting of patients who have been referred on a suspected cancer pathway, in line with nationalrequirements, from referral through to either non-malignant diagnosis or first and subsequent treatment for cancer, and ensure national operational standards of waiting times are met.

Main duties of the job

As an integral member of the Cancer Pathway Team, work with members of the Cancer Multidisciplinary Teams and administrative teams: Track and proactively manage suspected and diagnosed cancer patients, in line with national requirements, from referral through to either non-malignant diagnosis or first and subsequent treatment for cancer, and ensure national operational standards of waiting times are met.

Liaise with departments involved in providing care and core services to ensure planned treatment progresses smoothly and in a timely manner.

Ensure the patient tracking list is up to date and accurately reflects the stage of the patient pathway.

Obtain information via Trust Systems and input within timelytimescales ensuring accuracy in data documentation.

Uses judgment on escalation of patient delays and application of the escalation policy.
