Stockport Foundation Trust has an exciting opportunity for a Senior ACP to join the Directorate - Urgent Neighbourhood Services. Teams within this Directorate are Urgent Community Response, Hospital @ Home, One Stockport Care Navigation and Advanced Clinical Practitioners. This post will be pivotal within the clinical leadership and offer great opportunities to develop Advanced Clinical Practice
Main duties of the job
The post holder will practice as a senior clinician providing visionary leadership within Virtual Ward, UCR, ACP services and One Stockport Care Navigation that form the Directorate - Urgent Neighbourhood Services They will work alongside the GP Extensivists and form part of the senior clinical rota working over 7/7.The Senior ACP will practice independently and autonomously and assume a high level of clinical patients, seeing patients with undifferentiated and undiagnosed problems. The post holder will develop and support a team of Advanced Practitioners with the aim of improving the quality and efficiency of patient focused systems and processes. The post holder will also provide expert clinical and professional advice to staff within the Directorate, Division and across the wider organisation developing strong links with primary care and third sector organisations.

- Ensure the development and articulation of best practice within the service area and contribute across the service by exercising the skills of a reflective practitioner, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the field of Advanced Practice.
- Collaboratively work with MDT colleagues across the organisation and beyond to streamline pathways to support admission avoidance and facilitate discharge.
- Exhibit the highest level of interpersonal and communication skills and develop effective communication strategies with other members of the multidisciplinary team
The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.
If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
