Job summary

Job Overview:

We are seeking an experienced and highly skilled Advanced Clinical Practitioner (ACP) to join our dynamic Acute Frailty Team. As an ACP, you will play a critical role in providing timely, high-quality care to frail patients presenting with acute medical conditions.

This is an exciting opportunity to work autonomously, using advanced clinical skills to assess, diagnose, and manage a wide range of urgent and emergency presentations, supporting both admission avoidance and patient flow.

Main duties of the job

Responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision-making to patients, as per individual scope of practice, and through the advanced clinical competency framework..

Work autonomously within the specialty, managing a caseload of patients delivering individualised direct patient care.

Direct responsibility for assessment, examination, investigation and diagnosis of patients within their area of work.

Appropriately treat patients. Ensure the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.

Receive referrals including direct patient referral. Management of own caseload and clinical priorities according to agreed protocols and working practices.

Participate in multidisciplinary clinics; ward rounds, patient reviews and multidisciplinary team meetings.

Undertake a variety of clinical skills and provide treatment/advice as per speciality and individual scope of practice. Use agreed protocols for clinical practice and professional guidelines.

Undertake diagnostic procedures and clinical investigations.

Undertake a Non Medical Prescribing role and advice on medicine management issues associated with the patient specialty group. Work within Trust policy for Medicines Management.

Collaborative Working: Work closely with the multi-disciplinary team, including consultants, junior doctors, nurses, and allied health professionals, to ensure seamless, high-quality patient care.

Patient-Centred Care: Ensure all care is delivered in line with best practice, incorporating shared decision-making and patient involvement in their care plan.

Leadership and Supervision: Provide leadership and mentorship to junior staff, promoting the development of advanced practice skills within the unit.

Clinical Governance: Participate in clinical audit, quality improvement initiatives, and service development, contributing to the continuous improvement of the SDEC service.

Education and Training: Actively participate in the education and training of staff, students, and junior doctors within the unit, ensuring the dissemination of up-to-date clinical knowledge and practice.

Professional Development: Maintain clinical competence and continued professional development through reflective practice, attending relevant courses, and engaging in supervision and appraisal.

Qualifications and Experience:

Essential:

o Registered healthcare professional (e.g., Nurse, Paramedic, Allied Health Professional) with current NMC/HCPC registration.

o MSc in Advanced Clinical Practice (or equivalent).

o Independent prescribing qualification.

o Substantial post-registration experience in an acute/urgent care setting, including previous experience in emergency medicine, acute medicine, or similar areas.

o Proven competence in clinical assessment, diagnosis, and management of acutely unwell patients.

Desirable:

o Experience working within a Medical Same Day Emergency Care (SDEC) Unit or similar service.

o Experience in leading and managing service improvement projects.

o Experience in teaching, mentoring, or supervising junior staff.

Skills and Attributes:
