Job summary

The newly created Digital Health Service will be responsible for the clinical assessment and monitoring of Virtual Ward patients alongside the senior clinical assessment and care navigation of the acutely unwelladult that has been referred via the Urgent Single Point of AccessThe post holder will be responsible for managing the team which support remote monitoring of patients upon the Virtual Ward and offer senior clinical care navigation.The post holder will be responsible for ensuring that all digital platforms are operational and be compliantto the service standard operating procedure.The post holder will be responsible for ensuring the timely assessment and appropriate care navigation ofacutely ill patients that are referred via the SPOA Is completed using the appropriate clinical assessmenttool for triage and carer's needs.

Main duties of the job

In conjunction with the Pathway Lead - Out of Hospital, Urgent Community Services, provide effectiveleadership, monitoring of professional and care standards, and effective use of resources within theDigital HubTo operationally manage the Digital Hub team and deputize for Pathway lead when required.Support and develop health and social care integration across sharing responsibilities for leadership andmanagement of staff in multi-professional teams.To be a visible and accessible presence in the clinical area where appropriateTo be actively involved within the clinical care navigation and digital monitoring of patients within thecaseload.To act as role model and mentorTo understand the clinical pathways aligned to Virtual ward and ensure the deteriorating patient isappropriately escalatedTo ensure clinical competency frameworks are in operationTo be responsible for ensuring the provision of compassionate, safe and high quality patient careActively contribute to and support agreed developments within the Directorate with focus for supportingclinical effectiveness and innovative practice
