Advanced Pharmacist - Transfer to Assess/Complex Discharge Services

Job summary

The successful applicant will be responsible as an advanced specialist for leading and providing safe, efficient and cost-effective clinical pharmacy services to Transfer to Assess and Complex Discharge patients at Stockport NHS Foundation Trust.

Main duties of the job

  • To ensure rationalisation of medication particularly for complex patients with polypharmacy, including rationalising medication timing to aid patient concordance for self-managed medication, and to help reduce the number of care calls required for patients with managed medication.
  • To liaise with the community health care teams about the patient's needs including ongoing monitoring requirements
  • To provide support and medication advice to other members of the multi-disciplinary team
  • To use the skills as a pharmacist prescriber to write discharge prescriptions to meet service needs

  • To work across traditional boundaries e.g. primary / secondary care to provide seamless care.

  • To participate in the pharmacy departments on call and out of hours pharmacy services rota as and when required.
