Diabetes Specialist Nurse

Job summary

A fantastic opportunity has come up for a dynamic and enthusiastic qualified nurse to join the Stockport integrated diabetes specialist service.

The post will offer the right individual an opportunity to operate within all elements of the speciality and to gain valuable experience working alongside the multiprofessional team that assists within the individualised holistic care of patients with Diabetes.

You must be a qualified nurse with relevant experience in diabetes, either working in a specialist diabetes role or a role where you are able to demonstrate provision of enhanced diabetes care.

Ideally you will have undertaken an accredited course in diabetes and be able to demonstrate appropriate knowledge of relevant diabetes guidelines.You will also hold a teaching and/or mentoring qualification and demonstrate experience and knowledge in education, teaching and mentorship for health care professionals and patients. Applicants must be able to work on their own as well as being part of the multi-disciplinary team. You will manage your own work load and undertake nurse led clinics, multi-disciplinary clinics and home visits as required. You will play a key role in supporting and educating patients, as well as healthcare professionals to improve the quality of diabetes care.

Main duties of the job

Be professionally and legally responsible and accountable for all aspects of own work using evidence based practice and expert knowledge in the area of diabetes to influence the management of patients in your care (in accordance with National and local Trust policies and NMC guidelines).

Be clinically responsible for a designated caseload of patients with diabetes, ensuring high standards of care. Be prepared to undertake training in order to fulfil further duties identified as part of the role within service developments or individual performance review.

Organise workload effectively and efficiently with regard to clinical priorities and use of time, and in accordance with service demand/requirements on a changing basis.

Provide telephone assessment and support/education patients, relatives/ carers, including counselling and goal setting on risk factor modification, acknowledging instances where there may be difficulties due to language, education or other barriers, thus facilitating appropriate care/support on the phone.

Act as an educational resource for patients, families/carers and other healthcare professionals, with respect to all aspects of diabetes, its signs, symptoms and management.
