Colorectal Cancer Care Coordinator

FULL TIME POST - 37.5 HOURS A WEEK

An exciting opportunity has arisen within the Colorectal Cancer Nurse Specialist team as a Cancer Care Coordinator. The role will involve supporting the Clinical Nurse Specialist team with the delivery of Personalised Care for Cancer.

Personalised Care for Cancer includes supporting the provision of Holistic Needs Assessments, the development of a supportive care plan and offering Health & Wellbeing advice and support.

In addition to this, the role will also involve supporting the implementation and administration of Personalised Stratified Follow-Up monitoring and tracking of surveillance tests and being a single point of access for patients. This will include rapid re-entry into the system for those people identified as having urgent or specialist needs.

Main duties of the job

You will work closely with all grades of medical and nursing staff and wider health care teams. The role requires a team player but also someone who can work independently and use their initiative - it is an excellent development opportunity for someone who is enthusiastic with excellent organisation, IT and interpersonal skills and who is passionate about ensuring the best possible compassionate care for people living with and affected by cancer.

Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g. patients, relatives, carers, other health/social care professionals and community workers is essential.
