Job summary

Stockport Foundation Trust are looking for a dynamic, innovative Advanced Practioner with a wealth of experience to join the team. This will be a lead role as Consultant ACP that will continue to drive the Divisional agenda for Advanced practice.

The post will be structured around four core functions that exemplify the role:

o An expert clinician / practice function

o A professional leadership and consultancy function

o An education training and development function

o A research, evaluation and service development function

To be responsible for developing, implementing and reviewing the Advanced Practice ensuring that the service is delivered equitably and to a consistently high standard.

The post holder will be responsible for leading, developing and implementing innovative and creative models of care management for patients across community and assist in the implementation of urgent treatment centre models.

The post holder will utilise their skills in clinical examination, assessment and planning care for patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carer, as well as contributing to the operational and strategic developments to provide a more co-coordinated and equitable approach to advanced nursing across community services and will be responsible for maintaining and enhancing professional standards of care.

Main duties of the job

The consultant ACP will be responsible for leading, developing and implementing innovative and creative models of care management for patients within the community setting and assist in the implementation of and development of future models.

The post holder will utilise their skills in clinical examination, assessment and planning care for our patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carers, as well as contributing to the operational and strategic developments to provide a more co-coordinated and equitable approach to the specialty and will be responsible for maintaining and enhancing professional standards of care.

With the aim of improving the quality and efficiency of patient focused systems and processes the post holder will have responsibility of develop a robust strategy plan to modernise the ACP team and to provide a high quality, patient-centred service for patients. The post holder will also provide expert clinical and professional advice to staff within Community and across areas of the Trust.

The post holder will deliver a 65% clinical component to their role and 35% related to leading clinical practice, research and innovation, teaching and training.

Autonomously act and be recognised as a clinical expert in their specialist field using developed specialist clinical knowledge ensuring patient/client centred care and family involvement, including health promotion and disease/accident prevention.

Make difficult decisions and encourage critical thinking regarding both ethical and legal issues related to care delivery.

To manage own case load (whilst acting as an Independent Prescriber) that will often be complex and unpredictable: Receive referrals, assess, order, interpret and act upon investigative tests, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients/clients within the Trust

Provide support and supervision for senior staff working towards a higher level of practice.

Communicate regionally, nationally and internationally on area of specialist knowledge.

The post holder will provide expert specialist service development and advice to agencies within the Trust

Provide leadership for services developing a framework for professional development and service development.

To work collaboratively and strategically with representatives of other clinical areas, disciplines and services across the Stockport Neighbourhood Care to provide a seamless pathway of care

Represent the Trust at the local, regional and national level as required.

Actively engage in clinical audit for both national & local audit and critically appraise the outcomes of research, evaluation and audits and apply them to improve practice

Communication and working relationships

Ensure the development and articulation of best practice within the service area and contribute across the service by exercising the skills of a reflective practitioner, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the field of Advanced Practice.

Exhibit the highest level of interpersonal and communication skills and develop effective communication strategies with other members of the multidisciplinary team

Lead, facilitate, enhance and develop appropriate Advanced Practitioner skills and expertise through regular supervision in accordance with professional practice guidelines.

The post holder is required to provide, analyse and receive highly complex and sensitive information.

Use his/her expert knowledge and experience to provide professional advice and support to nurses, and other staff and managers across Community Services.

Challenge practice standards and influence constructive and positive change.

To ensure that accurate documentation and records of patient care are kept
