Job summary
The post holder will be responsible for ensuring that medicine reconciliations, on the assigned wards, are completed as soon as possible after admission. They will also be responsible for ensuring discharge prescriptions are dealt with quickly and efficiently, responding to medicines information queries and conducting audits
The rotational pharmacist will rotate through each speciality every 12 months.
To prescribe if appropriate for patients within the job holder's level of competence.
Main duties of the job
To provide, deliver and develop the clinical pharmacy service in accordance with agreed objectives
To co-ordinate the day to day activities in the assigned area
To act as a role model for clinical pharmacy practice and to provide training for medical, nursing and pharmacy staff.
To prescribe as appropriate for patients (including discharge prescriptions) within the job holder's level of competence
