Job summary
The post will be structured around four core functions that exemplify the role:
- An expert clinical practice function
- A professional and operational leadership and consultancy function
- An education training and development function
- A research, evaluation and service development function
To be responsible for developing, implementing, and reviewing the Integrated Care Advanced Practitioner Strategy ensuring that the services are delivered equitably and to a consistently high standard.
The post holder will be responsible for leading, developing and implementing innovative and creative models of care management for patients across Acute Medicine and Frailty services and assist in the implementation of Medical Same Day Emergency Care models.
The post holder will utilise their skills in clinical examination, assessment and planning care for patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carers. As well as contributing to the operational and strategic developments the role will provide a more co-coordinated and equitable approach to Advanced Practice across Acute Medicine and Frailty services and will be responsible for maintaining and enhancing professional standards of care.
Main duties of the job
The Consultant ACP will be responsible for leading, developing and implementing innovative and creative models of care management for patients within Acute Medicine and Frailty services and assist in the implementation of and development of future models.
The post holder will utilise their skills in clinical examination, assessment and planning care for our patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carers. As well as contributing to the operational and strategic developments to provide a more co-coordinated and equitable approach to the specialty, and will be responsible for maintaining and enhancing professional standards of care.
