Job summary

An exciting opportunity has arisen at Stockport NHS Foundation Trust for a Haematology Clinical Nurse Specialist. Acting as a key worker for patients and their carers you will deliver comprehensive specialist haematological care ensuring the highest standard of personalised care.

This post requires vision, innovation and the ability to work across professional boundaries underpinned by a real desire to provide the highest standards of patient care. Applicants for this post are invited from nurses who have extensive knowledge, skills and experience in Haematological Cancers, are enthusiastic, motivated and who have excellent communication skills.

Main duties of the job

The post holder has a key leadership and specialist role to support the organisation in the delivery of care for haematology patients and their families. This will be in accordance with the philosophy and standards of care within the specialty, ensuring that Trust values and beliefs are upheld. In addition, the Specialist Nurse has a fundamental role in leading on the development and the effective delivery of specialist cancer nursing to reflect the evolving health care agenda.

Job responsibilities

  • Acting in the role as a named key worker for patients and their families/carers
  • Use advanced communication and psychological assessment skills to assess and alleviate psychological distress facilitating compassionate conversations for cancer patients and their families, including referral to other agencies / disciplines as appropriate.
  • Delivery of Personalised Care to ensure everyone diagnosed with Cancer Unknown Primary receives a holistic needs assessment (HNA), personalised care and support plan and health and well-being advice throughout the pathway, signposting to support services as appropriate.
  • Be an advocate for the patient at haematology meetings by contributing to Multidisciplinary Team Meeting discussions and wider conversations regarding their care.
  • Work collaboratively with the Specialist Multi-disciplinary team at the tertiary centre.
  • Undertake agreed clinical procedures relevant to haematology, in accordance with the standard operating procedures, clinical protocols and /or patient directives.
  • Lead and develop nurse-led clinics/activity, including ordering investigations and interpreting results as appropriate for Haematology in line with competency training and trust requirements.

You will work in collaboration with the Haematology Consultants and other members of the multi-professional team to ensure the continuity of an integrated seamless holistic approach to Haematology Cancer Care. You will play an active part in clinical practice, education, research/audit and the implementation of national guidelines. Undertake holistic assessment and care planning, nurse led clinics and work as an integrated member of the cancer nurse specialist team.

Haematology services are currently being provided in in day case and ambulatory settings. Stockport provides both systemic anti-cancer therapy (SACT) and supportive therapies to a wide range of both malignant and non-malignant haematological conditions, using an out-patient model. The post holder will be integral to the quality service which is provided.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £48.43. This cost will be deducted from your first Trust salary.In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
