Job summary
The role of the Family Nurse is to work with in the Family Nurse Partnership Team to engage young parents; mothers and their partners in pregnancy and to deliver the personalised evidence based programme that has been developed by the FNP National Unit.
Family Nurse Partnership is an integral part of the Stockport Family Integrated service; the role involving developing effective working relationships within the wider service and optimising the outcomes for young parents and their children. There is a clear focus on promoting child development and school readiness.
The successful post holder will be required to attend residential training on 5th-9th February 2024 and 22nd and 23rd April 2024.
The successful post holder will be required to work Fridays, other working days can be negotiated.
Main duties of the job
To work in collaboration with local statutory and mainstream services to ensure that families become confident in accessing a range of other universal health and support services and are ready to graduate from the programme when the child is two years old or when ready to do so based on a collaborative assessment.
