Job summary

We are recruiting for a Youth Worker for Children, Young People and Familieswho shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

The role is full time, predominantly Mondayto Friday with some occasional weekend,evening,andcommunityworking. We are looking for someone withyouth work experience in the NHS, voluntary sector, or community services.

You will support young people with shorttermhealth needs as well aslong-termhealth conditions such as diabetes,andepilepsyandthose withdiagnosessuchasADHDand autism. The focus of the work will be on thechild'sandyoung person's social and emotional needs withinChildren's Services,Adultservices,and theCommunity.

You will be offering holistic advice and care toChildrenand YoungPeople(CYP)that are within Children's Servicesandthose who are being prepared for the move from children's to adult healthcare services. TheYouthWorkerwill deliver this work through one to one, group work, peersupport, and youth work sessions. You should be self-motivated,be able to usetheir owninitiativeand be responsive to the needs of youngpeoplein ahealthysetting.

Main duties of the job

Communicate effectively with CYPand their families/carers. Use acompassionate,individual,andinclusiveapproachto communication with childrenandyoungpeople who may have socialandcommunicationdifficulties,/special educational andlearning needs, learningdisabilityand autism.

To provide holistic support for the young personto meet their individual needs. Acting as an advocatetoCYP betweenmultipleagencies andservices, internal and external toENHTTincludingboth physical and mental health servicesandeducationandsocialservices.This requires good communication skills, active listening skills and to be empathetic towardstheCYP'sneeds.

Assess the needs ofCYP to correctly plan and deliver targetedprogrammesforexample forhealth-relatedconditions, fitness,smoking, drugs, gangs, violence, relationships and bullyingetc.

Work in partnership with families and other key people in theCYP'slife, as well as with professionals from otherorganisationsinvolved withCYPsuch as social care,physical and mentalhealth, police, education, youth offending teams and local authorities,in order tobuilda strong supportnetwork

Establish boundaries and challenge inappropriatebehaviour.
