Job summary

We are recruiting for Clinical Nurse Specialist - Hospital at Night Coordinators who share our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

You will be responsible for expert, safe, effective triage and coordination of all clinical tasks across all specialities and divisions, ensuring patients receive the right care at the right time securing positive clinical outcomes and increased efficiency. The HAN coordinators will also be expected to assess and manage acutely unwell and deteriorating patients and take part in clinical audit, education and present at appropriate.

Main duties of the job

Expert clinical triage and prioritisation of key clinical tasks

Effective and timely communication with teams across all specialities and divisions

Clinical assessment and management of acutely deteriorating patients

Develop the skills of generalist nursing staff and other members of the MDT to provide high standards of care for the deteriorating patient

Provide specialist support and advice to all stakeholders ensuring positive outcomes for patients

Communicate highly sensitive and complex information in dynamic and complex environments

Utilisation of software and systems to coordinate activity seamlessly

Clinical audit, evaluation of data and implementation of improvement projects and practices

Responding to calls made as per Martha's Rule

Key member of emergency response team

Close liaison with Critical Care Unit team, coordination of Critical Care discharges to ensure safe transfer to ward level care
