Job summary

We are recruiting for a Children's Community Support Worker who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

The post holder will work collaboratively and co-operatively with others to meet the needs of patients and their families, Participate in the development of self and others and provide individualised basic nursing care to a child/children in the community.

Main duties of the job

At the direction of the Registered Nurse the post holder will participate fully in the delivery of planned nursing care and undertake specific Trust approved clinical skills relevant to the delivery of care to children with special needs within the community, having undertaken appropriate training andassessment.

The Children's Community Support Worker will assist the registered Nurse in the evaluation of care and completion of patient documents. Involve patients and carers/relatives in the delivery of care, working in partnership with the family.
