Job summary
We are recruiting for a Specialist Palliative Care Administrator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
The post holder will be responsible for administrative duties as required in relation to both the Palliative Care Education Lead & the Lead Nurse for End of Life & Palliative Care.
In the absence of the Specialist Palliative Care Co-ordinator, they will also support the two Specialist Palliative Care multi-disciplinary teams (SMDT). The SMDT's are held in Lister Hospital and Isabel Hospice and the post holder will collate information relating to patients who have been assessed by the services in the last 7-10 days and who are to be discussed at these SMDT's.

Main duties of the job
As guided, the post holder will ensure the appropriate information is available for full discussion by all the members of the SMDT. The post holder will ensure that all the information required is recorded by a clinician during the meeting and then disseminated to the GP and other appropriate professionals if necessary.
The post holder will endure a smooth line of cross organisation communication.
The post holder will collate all referrals to the Specialist Palliative Care Services within E & N Herts on an electronic database.
The post requires initiative and a measure of independent judgement and will operate within procedural guidelines. Excellent communication and professionalism are essential, as this post holder will have a close working relationship with all professional groups across East and North Herts Specialist Palliative Care Services.
