Job summary

The Virtual Hospital at West Herts comprises of collaboration between West Herts and our community partners at CLCH. The purpose of the virtual hospital is to treat acutely unwell patients in the comfort of their own place of residence, by remotely monitoring them and providing home visits where necessary. Our vision is to support the patient's journey in a way that improves overall patient experience whilst also improving bed flow in the acute wards by preventing admission and supporting early discharge.

The role of the Patient Pathway Coordinator for Virtual Hospital is to support the VH hub team in looking after our patients by ensuring protocols are followed. You will be a key point of communication between our nursing and medical staff and the patient, as well as externally to GPs and other care providers.

Currently, the VH offers the following pathways - Heart Function (HF), Asthma, Bronchiectasis and COPD (ABC), General Medicine (GM), Frailty and lastly a small Post-Surgical Colorectal pathway.Not only do we provide specialist care for our patients, but we work to provide strong wrap around care to prevent readmissions..

Main duties of the job

Ensuring the patients pathway is protected by providing secure, timely, and appropriate arrangements in accordance with the VH pathways.

Maintain adherence to patient tracking process and referrals into the appropriate MDT at the requested times on the VH pathways. Also adhering to local/specialist MDT protocols and providing timely escalation of potential issues to the relevant clinician/AHP.

Providing effective and efficient communications with all internal and external members of the VH team.

Ensure that encounters, bed boards and patient records are updated correctly on Cerner.

Participate in data collection and be the link with IT to manage the IT systems associated to the VH.

Responsible for coordination, documentation, and follow up of administrative actions.
