Band 4 - Discharge Coordinator, Integrated Discharge Team

Job summary

We are looking for a motivated individual to work within the Integrated Discharge Team at Watford General Hospital.

The Discharge Coordinator is a Band 4 position full time hours at 37.5 PW. This is a secondment for 10 months. This also includes covering a 7 day service.

Main duties of the job

The Discharge Coordinator will act as the point of contact for the ward teams, AHP's, IDT and SPOC promoting an effective and smooth discharge for all patients on their designated wards.

They will be responsible for highlighting any referrals which the DPN will need to complete. They will then ensure that all aspects of the patient's discharge is progressed and escalate when there are changes or delays to the EDD, patient needs or clinical condition.

Education and qualifications

Essential

  • Evidence of a good standard of general education/GCSE or equivalent - Maths and English level c or above or equivalent
  • Basic IT skills
  • Evidence of ability to undertake job related training.
  • Willingness to undertake training to meet competency framework.
  • Evidence of relevant short course and in-service training.

Desirable

  • To have completed recent care certificate or be committed to complete on employment.
  • Knowledge of specific patient conditions, types of therapy intervention acquired through training and experience.
  • NVQ Level 3, health or social care qualification or equivalent

Knowledge

Essential

  • Evidence of knowledge of Hospital Environment and Pathways
  • Good knowledge of Health and Safety legislation and safe working practices.
  • Accepts responsibility for own workload.
  • Recognises the limits of own authority within the role
  • Seeks and uses support appropriately
  • Understands and practices the principles of confidentiality at all times.

Desirable

  • Knowledge of related legal frameworks for social and health care.
  • Knowledge of local health and social care services.
  • Experience of working with health/social care databases
