Job summary

Resident Doctor ST3-8 Level in Cardiology.

We are looking for a further full time post for a Resident Doctor ST3-8 level at West Hertfordshire Hospitals NHS Trust (WHHT).

These posts have been developed to support our expanding Cardiology department in order to have a full complement of 7 SpRs (4 Deanery StRs and 3 trust grade SpRs). As of November 2018, the Health Education England has written to us that there will be an increment of our Deanery StRs to 5 (making a total of 8) which reflects our commitment to training of SpRs.

Main duties of the job

The post holder will work as part of the West Hertfordshire Cardiology Department. Whilst currently not part of the formal Deanery Training Programme, this post offers an excellent training opportunity to junior doctors that have completed their core medical training and plan a career in Cardiology.

It will involve treatment of in-patients and out-patients and training in all main aspects of cardiology (echocardiography, CT coronary angiography, cardiac MRI, interventional cardiology and implantation of permanent pacemakers). For more experienced applicant there is potential for training in electrophysiology and implantation of complex devices.

Our current trust grades are working successfully towards accreditation in echo and MRCP qualification, presenting in international meetings and are encouraged to apply for deanery posts.

Job responsibilities

Cardiac Imaging

The department has a very busy echocardiology service. Transthoracic, transoesophageal and stress echo are all available most weekdays. There is daily coronary CT service run by cardiology and fully up and running cardiac MRI run by cardiology, currently working at three days per week.

Cardiac Catheter suite

Two Siemens cardiac catheter laboratories were installed at Watford General Hospital in 2009. There is an adjoining 12 bedded cardiac day ward. In 2015/16 approximately 2500 procedures were undertaken including diagnostic angiography, device procedures (brady. and complex rhythm management devices), percutaneous coronary interventions, and electrophysiology (diagnostic and therapeutic, now using 3D mapping) cases.

Coronary Intervention

Coronary angiography started in the Trust in 2005, in a single cardiac catheter laboratory at Hemel Hempstead Hospital. Coronary Intervention started in 2006, and Primary PCI (daytime, weekdays) in 2007. The invasive cardiac service was moved to Watford General Hospital in 2009, when two catheterisation laboratories were commissioned. Elective and acute coronary intervention is performed every week day. Most elective cases are done using radial access, almost all as day case procedures. Pressure wire, IVUS and OCT are used regularly. There is a weekly PCI meeting, where all planned interventional procedures are discussed between interventional and non-interventional cardiologists. Details about all interventional procedures are uploaded to the national PCI audit.

There are five cardiology nurse specialists, one dedicated to arrhythmia, two dedicated to heart failure, and two who manage the Rapid Access Chest Pain clinic, Phase 1 Cardiac Rehabilitation and PCI follow up. A business case has been approved to appoint additional nurse specialists, to expand the arrhythmia service and provide rapid access clinics for more conditions.
