Job summary
An exciting opportunity has arisen for an experienced practitioner to join the Division of Medicine here at West Hertfordshire NHS Teaching Trust as a Consultant practitioner for Care of the Elderly. The successful candidate must have a passion for care of the elderly and demonstrate a vision for the future.
Consideration will be made for someone who may not have all essential attributes and experience, however, is working towards gaining experience to lead in this role.
The post holder will act as an expert practitioner, and a source of expert advice to provide a clinical strategic nursing lead in developing pathways within care of the elderly at a local and regional level.
The successful candidate will work within the four pillars of advanced practice leading and supporting the delivery of specialist care to patients with complex needs. Specific areas will include Dementia, Parkinson's Disease, Continence, Frailty and Enhanced Care.
We are looking for a highly skilled, motivated, and dynamic practitioner to deputise for the Head of Nursing supporting the delivery of key objectives and strategic goals driving quality assurance and improvement in relation to specialist care of elderly and vulnerable patients.
The successful candidate will be innovative and someone who strives to deliver person-centered care. The post holder will need to work autonomously and be confident in their own decision making, utilising their knowledge and skills to deliver excellent care.
Main duties of the job
The postholder will be required to demonstrate the ability to prioritise workloads and use their own initiative.
The successful candidate will provide specialised clinical expertise, expert advice and leadership to patients, careers, trust and trust staff with a particular focus on our patients with Parkinson's Disease.
- The post holder will act as a practitioner and a source of expert advice.
- The post holder will be responsible in improving the care and services for patients within the Care Elderly Directorate.
- They will enable and facilitate change to deliver a modernised service promoting innovative ways of working that are patient centered using quality improvement methodology.
- The post holder will be a visible, pro-active and accessible leader, acting as a clinical resource and ensuring the sustained delivery of high standards of care.
- The post holder will provide education in the specialist area to improve patient experience and continuity of care.
