We have an exciting opportunity within our Cardiac CNS team delivering nurse led services for patients with heart failure, arrhythmia and chest pain.
Applications are invited for experienced cardiac nurses to join our dynamic cardiac nurse specialist team. This post is an ideal opportunity for an experienced cardiac rehabilitation nurse specialist to lead the team in the delivery of the cardiac rehabilitation service at Maidstone Hospital.
Main duties of the job
The post holder will be responsible for the delivery of the cardiac rehabilitation services, in the hospital, community venues and patients' homes. Patients are referred from a variety of sources, but primarily from an acute inpatient episode or from a tertiary cardiac intervention centre. The post holder will also liaise with commissioners in order to develop the service in response to changes in contracts, and ensure that robust audit data standards are maintained.
The post holder will empower patients and their carers to participate in the management of their conditions, thus aiming to improve the longevity and quality of their lives.
The post holder will provide line management to a small team, 1 X BAND 6 Nurse and 1 x Exercise Physiologist
