Job summary
The Planned Care Co-ordination Team is responsible for delivering comprehensive administrative services to enable outsourcing of West Kent CCG pathways into the Independent Sector.
The role of Patient Pathway Tracker is to provide an effective interface between MTW and the Independent Service Providers, ensuring a timely and effective service for patients who have chosen to be seen in the Independent Sector.
A good knowledge of the 18 week pathway is necessary to fulfil this post, with a good understanding of wider NHS relationships between GPs and the Acute NHS providers.
The post holder should be self-motivated and be able to provide good communication and organisational skills, demonstrating a flexible approach to duties. You will need to be able to work unsupervised and be able to deliver to tight deadlines. The applicant should be able to demonstrate an interest towards learning new skills.
Main duties of the job

- Monitoring and managing RTT patient pathways within the Independent Service Provider (ISP) setting
- Ensuring accuracy, providing necessary administration for the Planned Care Coordination Department
- Daily communication to the team lead, escalating any issues or concerns regarding patient pathway activity
- Remaining proactive and striving towards service development and improvements
- Building and maintaining good effective working relationships with the ISP's
- Data validation/cleansing
- To provide support to other PCC Team members with duties as and when required - including booking patients and answering telephones if needed
- Working collaboratively with a vast range of internal and external stakeholders to ensure service needs are met
- To manage the transfer of patient information in line with the Trust's policies and procedures for patient records and clinic processes.
