Senior Clinical Pharmacy Assistant - Discharge Support

Job summary

We are growing our team to support patient flow and efficient discharge across the Trust, and have an exciting opportunity for an enthusiastic, innovative and hard-working individual to join us as a Clinical Pharmacy Assistant to support the discharge process and join the clinical pharmacy team on the wards at Tunbridge Wells Hospital.

You will play a key role in working as part of the wider multidisciplinary team on the wards, and will be directly involved in contributing to exceptional patient care.

You will have experience of working in a healthcare/hospital or pharmacy environment, and you will hold the NVQ Level 2 in Pharmacy Services (or equivalent).

You will be self-motivated, disciplined, energetic and enthusiastic about providing excellent patient care, demonstrating the Trust's Values..

Main duties of the job

Main duties attached to this role will include:

  • Developing and providing ward-based dispensing initiatives. We are looking to roll this out across the hospital and this role is instrumental in the success of the service.
  • Cultivating patient engagement, undertaking locker/POD checks, requesting drug history sources for clinical team reference, and facilitating appropriate medicine information/compliance aids.
  • Facilitating safe disposal of unused medicines.
  • Processing returns for disposal and recycling with general ward medicine/pharmacy housekeeping.
  • Cold chain monitoring and review.
  • Communication with ward staff anddirectingenquiries to appropriate member of pharmacy team.
  • Discharge Medicines Service referrals
