Job summary

We are looking for an experienced Clinical Pharmacist with a special interest in lipid management and cardiovascular disease to work in a multidisciplinary team to provide tailored care for atherosclerotic cardiovascular disease (ASCVD) patients at three named Primary Care Networks (PCNs) in West Kent.

This position is on a fixed term contract for 1 year as part of a health equality improvement project and will support risk reduction and lipid management in primary care, linking in with Maidstone and Tunbridge Wells Hospital multi-disciplinary lipid management team.

It is an exciting time to come and work with us as we are expanding our services to support primary care, working with GP practices and other partners across our health and care system. The post holder will be required to travel across PCNs and the Trust.

We are looking for an enthusiastic individual who enjoys the challenge of working in a busy multidisciplinary environment and who can demonstrate great project initiative and excellent communication skills to both patients and other healthcare professionals.

Main duties of the job

You will be a team player with significant clinical experience as a registered pharmacist who is also an independent prescriber with excellent interpersonal skills. Experience in delivering pharmaceutical services to patients within a specialist area or delivering a clinical project with meticulous attention to detail is essential.

The post holder will deliver tailored lipid management and care to patients at risk of ASCVD and may also be requested to participate in educational or practice supervision to support our trainees.

Maidstone and Tunbridge Wells NHS Trust is a large acute hospital Trust in the county of Kent.
