Our Trust developed Ambulatory Emergency Care units at Maidstone and Tunbridge Wells in 2015.The units take direct referrals from our Emergency Department, Ambulance services and GP referrals. We provide holistic and person-centred care aiming to utilise same day emergency care to avoid and reduce the length of stay of patients
We are expanding our team to include an Ambulatory Emergency Care Specialist Nurse on each site supported by and working with the Lead Ambulatory Emergency Care nurse for the Trust. Please note the position may require the individual to work over both sites at different times. Currently the position will be Monday - Friday, however as services progress we will look at a 7-day service.
The post holder will be responsible for identifying appropriate patients in ED for the service and supporting timely transfers to the Ambulatory Emergency Care units.
The post holder will be responsible for accepting and triaging medical referrals from the community i.e GP, Secamb. They will liase with the medical team to ensure the correct pathway for these patients.
Main duties of the job
The post holder will manage a case load of patients and work collaboratively with different ward teams, specialities, Consultants and nursing. They require excellent communication skills and be able to adapt and work in an ever changing and pressurised environment.
The post holder will carry responsibility for flow through the department ensuring that patients assessment, management and discharge is in accordance to the services key performance indicators. They will also be responsible for supporting the development of software including Sunrise, the Teletracking system and ambulatory emergency care dashboard and ensure staff are trained in using these systems.
The post holder would be autonomous and work with the expanding ambulatory emergency care team. This role requires dynamic individuals who have a passion for ambulatory emergency care, who are compassionate and able to problem solve and advocate for patients. As our service develops the postholder may consider a clinical Masters qualification to enhance skills in assessment, diagnostics and decision making, alongside the non-medical prescribing course. This would be subject to approval and funding.
The post holders will work with the Lead Ambulatory Emergency Care nurse to support education and development of staff across the Trust. They will also support with auditing services and quality improvement initiatives.
