Job summary
An exciting opportunity has arisen to join the ward clerk team on the new Planned Medical Day Case Unit at the Glenfield Hospital
The post requires you to have excellent communication skills and customer service abilities as you will be patient facing and dealing with patients and their relatives. You will be expected to meet and greet patients on arrival. You will be responsible for communicating with the nursing team within the unit, ensuring clinical notes are available. The candidate will ideally have experience with patient centre, HISS and be competent with computer skills as this role will require the applicant to co-ordinate clinics, booking in and discharging patients. Previous experience as a ward clerk within an NHS environment would be an advantage, however training can be given. A working knowledge of Trackit, Nerve Centre and Patient Centre would be beneficial.
You will be require to work using your own initiative, managing your own workload as well as being part of the Planned Medical Day Caseteam.
You will be expected to cover a rota that covers Monday to Friday 8am-6pm on a rota basis, ensuring that the needs of the unit are covered. This will be reviewed as the service develops.

Patient Information
- Using the Trusts IT system, ensure that the patients case notes and documents are available on the unit for the patients
- To make up temporary file for patients admitted direct to the ward i.e. emergency admissions or out of hours for Admissions
- Complete front sheets of notes showing dates of admission and
- To maintain all filing up-to-date.
- Ensure that patients notes are complete for ward
- To book in and out medical notes on the Patient Centre system training will be arranged.
- To trace patients on Patient Centre. If the patient cannot be found, register the patient using the completed admission
- To source and retrieve patient case notes from within the Trust and from other hospitals, liaising with the Medical Records Department as appropriate, checking and updating patients and GP details when received, to ensure accuracy and minimise errors.
- Ensure addressograph labels are available and attached to all medical and nursing notes, charges and prescription
- Enter accurate and timely patient information on Patient Centre ensuring accurate clinical
- Obtain patients notes required for clinics and doctors
Receptionist Function
- Act as the ward receptionist to all visitors; give directions and advice as
- Answer telephones, staff bleeps and bleep medical staff as
- Issue car park letters to parents/relatives as
Patient Flow
- Check and update the ward bedstate using Patient
- Ensure nerve centre is up to date
Discharge
- Ensure that discharge summaries (written by Doctors) are completed
- Ensure that all the patients documents are complete on discharge and ready for collection to medical records
- Discharge patients as required on Patient centre
- The Ward Clerk will be responsible for the co-ordinating and the booking of all new appointments
- To file all nursing and medical documentation in patients notes in the correct order on discharge and merging notes where
Administration Function
- To act as a Ward Receptionist by dealing with enquiries from patients, relatives or friends under the direction of the Ward
- Manage all incoming communications (e.g. telephone calls, internal/external emails and post to ensure that information is passed on clearly and to the relevant Ward
- To answer the telephone and staff bleeps, ensuring that all enquiries about patients are passed on to the Nurse in Charge
- To collect and take ward mail to the Post Room as
- Open all ward mail unless marked confidential and give to the Nurse in
- To cover annual leave/sickness on other wards/departments as required within the CMG
- To keep the notes trolleys clean and
- Attend ward clerk meetings as necessary
- Maintain an effective filing and bring forward
