Job summary

Due to the continued success of UHL's Frailty Same Day Emergency Care (SDEC) pilot, we are excited to offer opportunities for motivated and experienced Band 6 Physiotherapists to join our innovative Rapid Assessment and Discharge Service (RADS) team.

Frailty SDEC is a forward-thinking model of care designed to deliver timely, multidisciplinary assessments and interventions, outside of the Emergency Department, to older adults presenting with frailty, enabling same-day discharges and reducing unnecessary admissions.

Main duties of the job

As part of the RADS team you'll play a key role in delivering high-quality, person-centred assessments across the Emergency Department, Acute Assessment Units as well as Frailty SDEC. You will work collaboratively with a large multidisciplinary team (MDT) to provide comprehensive care supporting safe, timely discharges with robust therapy input and community liaison. Furthermore, as a team we are passionate about promoting independence and preventing deconditioning for all patients across the Emergency Floor through early therapy intervention, effective MDT working and teaching.
