Job summary
An exciting opportunity to provide clinical expertise as part of the University Hospitals of Leicester Community Liver health Checks Programme. Working with a range of community services across the Leicester and county region, the role will support the early detection of patients with fibrosis and cirrhosis, helping identify patients with early signs of Liver Cancer. Working alongside the Hepatitis nursing team, as well as Hepatology, Diabetes department, the role will include fibroscans of patients with significant risk factors associated with increased liver health problems. The post holder will be responsible for the management and treatment of patients identified with cirrhosis of the liver and onward referral to Hepatology.
Main duties of the job
- Manage a caseload of patients with significant liver health risk factors, delivering individualised and personalised direct patient care.
- Conduct and report upon routine and urgent Fibroscan needs of patients with significant clinical risk factors.
- Provide clinical expertise, specialist advice and support across within the hepatitis team.
- Support management with the development of liver screening provision linked to community support services, including but not limited to: Primary Care Networks, drug and alcohol services, high risk population groups, co-ordinated screening alongside other departments such as Diabetes
- Adopt a range of the Chief Nursing Officer 10 key roles appropriate to client group in order to improve and streamline patient care and patient experience.
- Demonstrate in depth specialist knowledge, skills and innovative practice in the specialist area, leading the delivery or appropriate evidence based care and advanced practice
- Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient care in the community.
- Analyse and interpret complex needs/care for patients.

The post holder will be responsible for the management and screening of patients identified with significant risk factors of related to liver health, fibrosis, cirrhosis and Hepatocellular carcinoma through the use of Fibroscans and onward referral to Hepatology services.
In this role, you will be responsible for :
Communicating and receiving complex and sensitive information in relation to fibrosis and cirrhosis of the liver and supporting patients to understand information/results
The disclosure and explanation of the diagnosis of fibrosis and cirrhosis, the patient subsequent patient to perform individualised ongoing care pathway planning and implementation / coordination of care for this client group.
To act as a clinical expert and professional resource for patients, their carers/families and the multi-professional team, acting as a role model and clinical resource demonstrating high standards of care and providing clinical leadership in the specialist area.
The post holder will be responsible for co-ordinating the requesting and reviewing of baseline investigations, and onward referral to hepatology services and to oversee the subsequent care of patients through face to face or virtual clinic formats.
The post holder will work flexibly alongside the lead viral Hepatitis nurse specialist, Hepatitis C nurses and Trust Peers to ensure the development and delivery of a high quality service for patients at significant risk of liver health issues in the community throughout the week, to include evening and weekend flexible working as the service requires
This post requires excellent communication skills, and the ability to work autonomously, and as part of a team as the service requires with the support of their line manager to develop and deliver a high quality service.
