If yes, we have an exciting opportunity for you working as a clinical aide in diabetes in our friendly supportive dynamic diabetes team.
We are looking for an enthusiastic, motivated and proactive person with excellent people skills to drive and support people living with diabetes. You will need to feel confident in finding different ways to communicate to people with diabetes who may have physical, cognitive and communication difficulties. We also use technology to enshace patient care. you will be a motivated person who is able to support people to manage such devices in the home environment and link directly with the diabetes virtual ward.
You will be working with the diabetes virtual ward team and the diabetes inpatient team, and will include cross site working

Main duties of the job

You will be working with a team of diabetes specialist nurses and clinical aides who will fully support you and provide you with training to enable you to fore fill the role.

You will be monitoring patient's conditions by taking their clinical observations, venipuncture when indicated.

Assist in the delivery of patient care as directed by the specialist nurse you will be working with.

Ensure that the patients are the prime focus when you are supporting the team with decisions about their care and well-being.

Assisting patients with compassion when undertaking normal activities ensuring privacy, dignity and human rights are respected.
- The role will involve supporting people with diabetes with managing their condition - supporting patients to take insulin, provide education, and use a wide range of devices to help monitor their condition.
- Support people with diabetes with technological devices to support patients to be supported post discharge on the diabetes virtual ward, along with supporting patients at home to self monitor and feed information into the diabetes virtual ward.
