Leicester Renal Community Team

An opportunity has arisen within the Leicestershire Renal Community Team, for a full time Band 5 Nurse. You will need to be a 1st level registered nurse with at least 12 months post registration experience with either renal or community experience.For the post, you will work as part of a team of nurses planning and delivering high quality care for a variety of patients across Leicestershire. The work will include the training and the ongoing care of patients on home therapies. You will be fully supported by the Team Leader and other staff within the team. The primary focus for the successful applicant will be to support the home haemodialysis lead in training patients to carry out their own dialysis treatments at home.The Renal Community Team is an innovative and forward-thinking area and offers a unique opportunity to further develop specialist nursing skills. The applicant should be interested in teaching and supporting patients to self care.Evidence of continuous professional development within the last 12 months is essential.A flexible working policy is offered to meet the needs of both the service and staff. The service is currently delivered Mon-Fri 8.30 to 16.30.Able to travel to patients homes and UHL hospital sites.

Main duties of the job

Responsible for assessing, planning, implementing and evaluating programmes of evidenced-based nursing care to a group of patients.

Provide advice, support and information to patients and their family or carers as required and within the limits of the post holders knowledge and experience. To Liaise closely with senior members of the community team.

Assisting and contributing in the management and organisation of a community team in the clinical area and community setting area.

Actively contribute to setting and maintaining high standards of quality nursing care.

Actively participate in the education, development and supervision of other staff members.

Act as a clinical and professional role model, assessing and supporting students and learners on placement within the clinical area.

Work collaboratively and co-operatively with others to meet the needs of patients and their families, ensuring the health education and health promotion needs of patients are identifies.

Organise and perform investigations for patients in the community setting, support staff members is providing a 'drop-in' renal day case service and training undertaken on the unit in the training room in accordance with directorate policy and protocols.
